Thank you for your interest in volunteering with Adaptive Mobility Providers, we are excited to have you participate with our dedicated team supporting athletics for all.

Please review the following items and complete ALL forms found below as they are required. We look forward to having you join us for the upcoming season. If you have any questions, please send an email to: info@adaptivemobilityproviders.org

STEP 1 - Volunteer Handbook - please download and review, once complete, please fill out the Acknowledgment Form.

STEP 2 - SafeSport Policy Handbook - please download and review, once complete, please fill out the Acknowledgment Form.

STEP 3 - Volunteer & SafeSport Handbook Acknowledgement Form - Please complete this form acknowledging receipt of Handbook.

STEP 4 - NCSI Background check - If you completed the NCIS background check in 2025 with AMP, you are all set. If not, please do so now.

STEP 5 - AMP & Move United Release Forms - If you completed these releases in 2025 with AMP, you are all set. If not, please do so now.

STEP 6 - ***COMING SOON *** SafeSport training - If you have taken the SafeSport training in the last 2 years, you only need the refresher course. If you have not completed the SafeSport training in the last 2 years, please complete the entire core training module.
